Additionally, Longevity Health Plan of Colorado, Longevity Health Plan of Florida, and Longevity Health Plan of Michigan, each received 5 Stars for their performance on Part D, while Longevity Health Plan of New York received 4.5 Stars. In addition to all rated plans achieving four (or more) stars, this recognition places Longevity Health among an elite group of plans nationwide. Out of 720 total plans rated by CMS, only 2.5% received an overall 5-star rating, highlighting the strength of Longevity's clinical and operational excellence.
