TUSK Practice Sales Releases Q4 2026 Dental Market Report on DSO Expansion, Tighter Financing and Exit Planning

PR Newswire

New market report recaps Q3 and the year-to-date, and explains what each shift means for dentists weighing a transition

CHARLOTTE, N.C., Oct. 14, 2026 /PRNewswire/ -- TUSK Practice Sales, the premier sell-side healthcare M&A advisory firm, today released its Q4 2026 Dental Market Report. The report is written for dental practice owners in the United States seeking to learn about the latest DSO and private equity activity, the macroeconomic and lending environment, specialty-dentistry trends, and the ownership shifts that will shape exits over the next five to ten years.

Key Findings

  • DSOs and private equity groups are expanding. DSOs and private equity groups entered new states throughout 2026, including a Canada-based group entering the US for the first time. The global DSO market's projected annual growth rate of 17.4% outpaces the US dental services industry's 4%.  State-level regulatory changes, such as North Carolina's Session Law 2026-41, are also lowering administrative hurdles for management arrangements.
  • More DSOs are preparing to go to market. PitchBook's Q2 2026 healthcare services report found 20 or more PE-backed dental companies held for seven years or longer. TUSK Practice Sales anticipates a resurgence of M&A activity once new capital enters the market.
  • Financing is tighter and costlier. The Federal Reserve raised its target range to 3.75% to 4.00% on September 16th, and new SBA underwriting rules took effect October 1st, adding pressure on associates and new owners who finance a practice purchase.
  • Dental practice ownership is changing. Dentist practice ownership has fallen from 85% in 2005 to 73%, DSO affiliation has more than doubled since 2015, and the class of 2025 entered practice with an average of $297,800 in education debt.
  • Dental specialties are growing at different paces. Pediatric dentistry has grown 126% since 2001 against 25.5% for dentists overall, and OSOs (orthodontic service organizations) are showing more interest in multi-specialty practices with orthodontic and pediatric lines

"DSOs are expanding, with the global DSO market projected to grow 17.4% a year through 2032, and they are going to continue to do so. In today's difficult lending environment, many associates and recent dental school graduates are waiting longer to become owners. The 33.8% of dentists aged 55 and up will need to consider expanding their options to include a full or partial sale to a DSO," shared Kevin Cumbus, President and Founder, TUSK Practice Sales.

Taken together, the findings describe a market where deals are still getting done and every dental practice owner has transition options, but where the path to each one is shifting. Operating costs are rising faster than payer reimbursement, with reimbursement up 1.1% in the first half of 2026 against 1.8% inflation, and financing is costlier for the associates who have traditionally bought in. The report also includes an explainer on what an IPO means for doctors who partner with a public dental company as more platforms consider that pathway. TUSK Practice Sales advises dental practice owners five to ten years from an exit to map every option now.

"Owners five to ten years from an exit have more paths available than ever, and more variables to weigh. DSO expansion is real, but so are tighter lending and a wave of groups preparing to go to market. We encourage all dentists to educate themselves on the value of their business and speak to their advisors, as we enter a new era of dentistry," concluded Ryan Mingus, Managing Director and Partner, TUSK Practice Sales.
